Forms to bring to Gainesville
After you have been accepted, you will receive notification by e-mail and mail of your acceptance. You should read all of the information in the files below and fill out the necessary forms. Bring the forms with you when you register at Beaty Towers.
One exception: The Travel Information form must be faxed before the workshop starts to (352) 846-2673. The mailing address, phone numbers, and e-mail addresses are in the what to bring letter (below)

- Behavior contract (read first)
- Behavior contract (form to sign and bring to Gainesville)
- Travel form (return only if using bus or airplane or if ordering a vegetarian meal for the opening night)
- Liability form (must be signed and notarized; bring to Gainesville)
- Medical form (must be signed; bring to Gainesville)
